FAMILY SOCIAL SERVICES PROGRAM MANAGER JOB DESCRIPTION

Family Promise of San Gabriel Valley (FPSGV) provides a holistic network of support for homeless families as they work to achieve and sustain independence. Bringing together the resources of faith communities, service agencies, and the local community, we provide a variety of services for families in need. FPSGV’s mission is to help families challenged with homelessness and low income achieve sustainable independence through a community-based approach.

FPSGV is currently seeking a program manager with excellent track record managing and developing programs. The Program Manager will have oversight of Family Promise of San Gabriel Valley Programs: Congregational Network Shelter Program (CNSP); Homeless Prevention Initiative, and other programs as they are developed. The Program Manager will be under direct supervision of the Executive Director to provide comprehensive case management, assessments/referrals, intake screening. 

Supervision: The program manager reports directly to Executive Director  

Essential Functions and Responsibilities

·      Comprehensive case management: outreach, intake, assessment, individual case planning, referral, housing assistance, advocacy

·       Provides weekly case management for families within the CNSP program

·       Provides monthly case manage for families participating in Homeless Prevention Initiative

·       Provides aftercare for families exiting program on a monthly basis

·       Responsible for accurate and up to date case files

·       Collects, reviews, and prepares monthly and yearly program data reports

·       Ensures client access to HOPE Day Center as appropriate

·       Attends all relevant meetings to position, i.e. weekly program meetings, bi weekly staff meetings, host coordinator meetings, community housing related meetings and other meetings on an as needed basis. 

·       Participates in the development of new programs and manages other programs as they are developed

·       Manages program budget and provides report on program spending 

Minimum Work Experience and Qualifications

·       2-3 years’ case management working experience with family homelessness and/or child and family services

·       Experience working with low-income families and homeless families from diverse cultural and ethnic backgrounds, and those with a history or currently experiencing mental health conditions

·          Knowledge of evidence based practice models, including trauma informed care and motivational interviewing

·          Ability to relate and work with faith-based congregational organization

·          Required Licensure, Certification, Registration: Master’s Degree in Social Work from a CSWE Accredited University

Preferred Work Experience and Qualifications:

·       3-4 years working experience with family homelessness and/ or child and family services,

·       2-3 years working experience in volunteer management;

·       Bilingual (Spanish/English)

·       Knowledge of Coordinated Entry System and HMIS, Housing First, Rapid Re-housing, harm reduction strategies, homeless services, and housing subsidy programs

·       Required Licensure, Certification, Registration: Master’s Degree in Social Work from a CSWE Accredited University

Additional Requirements

·        Able to lift up to 15 pounds (e.g., accepting donated items, carrying office-related supplies, etc.).

·       Knowledge of basic computer software (Microsoft Suite) and electronic communication tools.

·       Possess a valid Class-C driver's license, personal vehicle, proof of car insurance, and good driving record.

Working Hours

The Program Manager is a full time exempt position requiring some weekend and evening hours. After hours the Program Manager is on call for emergencies. Due to the nature of this position, flexibility of work hours may be required.

Salary

Salary for this position ranges between $48,000 to $50,000, plus health stipend.
